This Software may not, in whole or in any part, be copied, reproduced, transmitted, translated (into any language, natural or computer), stored in a retrieval system, reduced to any electronic medium or machine readable format, or by any other form or means without prior consent, in writing, from Compliance Specialists, Inc.
You are granted a limited license to use this software. The software may be used or copied only in accordance with the terms of that license, which is described in the following paragraphs.
"The software" shall be taken to mean the software contained in this package and any subsequent versions or upgrades received as a result of having purchased this package. "buyer" shall be taken as the original purchaser of the software.
Buyer has the non-exclusive right to use the software. Buyer may not use the software for any practice except that which a subscription was purchased for. Buyer may not modify or translate the program or documentation. User may not disassemble the program or allow it to be disassembled into its constituent source code. Buyer's use of the software indicates his/her acceptance of these terms and conditions. If buyer does not agree to these conditions, buyer must cease use of the software immediately.
This license agreement shall be governed by the laws of the United States of America, and shall inure to the benefit of Compliance Specialists, Inc. or its assigns. 
The materials and information contained herein are designed to offer basic information for promoting government regulation-compliance in the small health care private practice environment. They have been compiled based on the experience, training, and interpretation of their authors using Centers for Medicare and Medicaid Services (CMS) Guidelines, regulatory information from CMS and the Office of Inspector General (OIG), recent revisions to incentive programs for Medicare- eligible providers, and other coding and billing guidance.
Compliance Specialists, Inc. warrants that the information contained herein has been carefully researched and checked for accuracy and completeness. Compliance Specialists, Inc. does not warrant it to be free of errors or omissions, that government agencies will always agree with our interpretations of the applicable regulations, or that following the guidelines presented herein will ensure complete compliance with government regulations in all cases in the future.
